Westmed is continuing its commitment to quality care and innovation with an official launch of its first Remote Patient Monitoring (RPM) program. The program is named My Westmed Connect, and is a service offered to Westmed patients with chronic conditions created to help them better monitor and take action on their health between office visits. The program leverages a combination of Bluetooth-enabled devices – like blood pressure cuffs, glucometers, scales and spirometers – and a team of virtual Care Coaches who provide an added layer of support to patients on their personal health journeys.
My Westmed Connect offers eligible patients a free Bluetooth device to use while active in the program, which allows health data to transmit directly to the Westmed care team whenever a patient takes a reading. Each patient will also be assigned a Care Coach, whose role is to assist with set up, education on chronic conditions, medication adherence and general health and wellness tips. Patients will utilize an easy-to-use mobile application when performing their readings and access useful information with a goal of improving health outcomes.
